Emergency services worker dies after fainting in Costco fuel tank
An environmental services worker in Sacramento, California died this week after passing out inside a tank at a Costco store, according to local fire officials.
The man has been identified as Ashkan Moghaddam, 37, the Sacramento County Coroner's Office confirmed to USA TODAY.
Moghaddam had been trying to clean an empty underground fuel tank at Costco the morning of Monday, Sept. 8 when he fainted, Captain Justin Sylvia of the Sacramento Fire Department told USA TODAY. To get inside the tank, he had to go through a 3-foot round hole and climb 10 to 15 feet down.
Someone called the Sacramento Fire Department about the incident, Sylvia said. Within 15 to 20 minutes, units arrived and pulled the man out of the tank, the fire department added.
According to Sylvia, first responders found him unresponsive in the tank. First responders performed CPR and took him to the hospital, where he was later pronounced dead.

According to the fire department, the Occupational Safety and Health Administration (OSHA) is investigating the incident. It is unclear what caused Moghaddam's death.
USA TODAY has reached out to OSHA on Wednesday, Sept. 10, but has not received a response.
Who was Ashkan Moghaddam?
A LinkedIn page for Moghaddam shows that he had worked for Ponder Environmental Services Inc. since 2022. He previously worked for Underground Construction Co. Inc. in 2021 and for Patriot Environmental Services in August 2017.
USA TODAY contacted Ponder Environmental Services Inc. on Sept. 10, but has not received a response.
According to those who knew him, he was a father to two daughters. Warren You, who said in a Facebook post that he had known Moghaddam since preschool, shared photos of him competing in martial arts.
"We shared many adventures growing up, and the last time I saw you was at a BJJ tournament where I happened to capture photos of you doing what you loved," You wrote, calling Moghaddam "Ash."
Facebook user Joe McGuffin shared online that he was "an incredible individual." Calling him a friend and brother, McGuffin said in his Facebook post that Moghaddam "always had a smile for everyone."
"He was always the calm in the storm," McGuffin wrote. "He deserved better than the situation that befell him. I do and always will hold Ash in high regard. I only regret not being there when he was in need. He would be here with us today, if i was there. I will miss you my friend."
