After actress Isabelle Tate died Oct. 19 at age 23, *9-1-1: Nashville* paid tribute to the rising star with an on-air memorial during the show's latest episode on Thursday.

At the end of the episode, a title card appeared with words honoring Tate's life.

"In loving memory," the tribute read in honor of Tate, and listed her full name as well as her years of birth (2002) and death, after the actress first appeared on the pilot of *9-1-1: Nashville *that aired Oct. 9, with filming having taken place in June.**

"Isabelle 'Izzy' Tate had a rare form of Charcot-Marie-Tooth disease," Kim McCray, founder of the McCray Agency, told EW via email.

McCray said Tate "passed away peacefully," and that "the family requests privacy as they deal with this sudden and shocking loss."**

The representative concluded her note by stressing that "in lieu of flowers, the family asks that donations in Izzy's memory be made" to the Charcot-Marie-Tooth Association in Tate's name.

"Isabelle was full of fire, a fighter, never once making excuses for the fact that she might have a disability relative to others. She was also quite musically inclined, often spending hours writing and recording songs with friends and even publishing a few," read an obituary for Tate.

The Mayo Clinic explains that Charcot-Marie-Tooth is a group of "inherited conditions that cause nerve damage," primarily to the arms and legs.

Country singer Alan Jackson recently revealed he has Charcot-Marie-Tooth, and was first diagnosed in 2021. He announced earlier in October that, due to the condition, he would perform his final concert on June 27, 2026.

*9-1-1: Nashville* — an offshoot of the Angela Bassett-led *9-1-1 *original — stars Chris O'Donnell, Jessica Capshaw, and country singer LeAnn Rimes.

"Every time they tell me about the next crazy emergency, I think 'There's no way,'" O'Donnell told EW of the spinoff's premise, which, like the original and the recently concluded *9-1-1: Lone Star*, revolves around first responders and the wild emergency calls they receive.
